The Hu-Tech team has extensive expertise in Learning From Incidents (LFI) and how to use near –misses and incidents as opportunities to learn and improve safety defences. 

​Human Factors are often at the root of incidents and other failures to deliver safety. 

We combine this with our auditing capability to stress-test safety defences, control of work (PTW and supervision) and safety critical tasks. 

We use a single defences based approach to identify issues flagged by incidents, near-misses, audits and inspections. These are linked to improvement actions tied to specific safety defences and KPIs. In this way improvement can be both assured and sustained.
