Hu-Tech use bow-ties to optimise the human element in operational risk controls,  particularly to highlight the issues and solutions to risk management in Oil & Gas, Healthcare, Rail, Highways and COMAH scenarios.

Project work has included:
  • Identification and management of Safety and Environmental Critical Tasks
  • Identifying and avoiding surgical and medicines errors
  • Highway Traffic Control Management systems to prevent injury from incursions 
  • Passenger injuries transitioning between carriages and platforms

Bow-ties are a tool to represent the risk control measures in a graphical format.  Bow-ties are not a theory or model of risk control, the underlying model is that of threats, defences and trajectories.

What the bow-tie does is enable us to visualise:
  • Defences to threats prior to the event and escalation after it.
  • Multiple threats to be represented against a single hazard
  • Defence degradation
  • The mitigation of defence degradation
  • Multiple bow-ties to be linked
  • Attributes to be attached to elements in the bow-tie – primarily for defences and mitigations to degradation.
  • The inclusion of human elements such as ownership, tasks, competence and accountability
  • The weighting of elements.
